1) employment certificate (known as the credit certificate)
  • 2 copies to print
  • sign as at the departure date of the employee, and have the former employee sign an acknowledgement of receipt with that date
  • the employee takes one signed employment certificate; you keep the second (please send us a scan or upload it to the drive)

If you cannot meet the former employee in person, you can send the employment certificate by registered post with acknowledgement of receipt. Keep the posting receipt as proof of dispatch. If the employee accepts the letter, you receive the signed acknowledgement confirming delivery. If they do not collect it, deemed delivery applies after 10 days – the letter is considered delivered after 10 days.

2) income certificate
  • print 1 copy
  • sign and give it to the former employee

3) pension insurance record sheet (ELDP)
  • print 2 copies
  • both you and the former employee sign (including the date – the law requires the ELDP to be given to the employee no later than the day it is sent to the Czech Social Security Administration)
  • the former employee takes one signed ELDP; you keep the second (please send us a scan or upload it to the drive)
